Once you have committed to and/or signed with VU, please read and understand each form below before completing and signing. Once completed, your coach will receive an electronic copy for review. All forms should be completed prior to participation in any sanctioned Vincennes University athletic activity.

This form verifies a student-athlete’s eligibility to compete under NJCAA rules at VU.
Sexual Harassment, Hazing, & Bullying
This form outlines VU and the Department of Athletics’ policies on sexual harassment, hazing, and bullying, all of which are strictly prohibited.
Sports Media Profile & Release
This form gathers personal, athletic, academic, and promotional information for new and returning VU student-athletes to support media guide bios, team promotion, and public recognition.
ATS Account Creation
Creating an ATS account gives VU student-athletes real-time access to our Sports Medicine Staff, ensuring comprehensive care throughout their time at the University. The account also contains medical forms that must be completed to certify participation and ensure the highest level of medical care. For instructions on how to complete your account, click here.
Physical Examination Form (Do Not Complete)
This form will be completed once you arrive at VU, as part of our yearly Department of Sports Medicine protocols. Your yearly physical examination must be completed at VU by our Department of Sports Medicine staff in order to participate in VU Athletics.
